OUR GOAL: The goal of the Family Support Program is to assist individuals with severe disabilities and their families to remain together in their homes and communities.
OUR FOCUS: The primary focus of the Family Support Program is to support families with children with severe disabilities, school age or younger, adults with severe disabilities that choose to live with their families and adults with severe disabilities who are residing in the community in an unsupported setting(not a state or federally funded program.) The program is funded by state dollars and helps families purchase services, supplies, and equipment. Services may be limited by the availability of funds and local priorities.
OUR SERVICES: Some examples of services that are eligible for reimbursment are: Respite Care, Day Care, Homemaker Services, Specialize Equipment, Specilized nutrition, clothing, and supplies, Personal Assistance, Transportation Services, Health Related costs not otherwise covered, Minor Home and Vehicle modifications, Maintenance and repairs to specialized equipment, Summer Camp for children and adults, and Housing Costs.
WHO IS ELIGIBLE: An individual with a ongoing severe disabilities that results in major functional limitations in at least three activities of daily living.
COUNTIES SERVED: Pacesetter's Inc. administers the Family Support Program for the following 12 Counties in the Upper Cumberland area: CANNON, CLAY, CUMBERLAND, FENTRESS, JACKSON, MACON, OVERTON, PICKETT, PUTNAM, VAN BUREN, WARREN, AND WHITE.
This project is funded by a grant with the Tennessee Department of Intellectual and Developmental Disabilites.
